1095-C Forms Now Available

Each year, Triage is required to send out 1095-C forms to show that health insurance is offered to each employee. You’re not required to do anything with this form, but you should file it with your tax documents. Everyone will receive this form, even if you declined Triage health coverage.

Your 1095-C form is now available in UKG. To access your documents online, please visit UKG (use the website, not the app).

A copy is also mailed to the address on file with UKG, if elected.
