What We’re Looking For

The traveling Case Manager coordinates patient care across the healthcare continuum, ensuring optimal patient outcomes, efficient resource utilization, and seamless transitions of care. This role demands a highly adaptable professional capable of quickly integrating into diverse healthcare settings and protocols while managing complex caseloads.

Requirements

  • A Bachelor's or Master's degree in Nursing (BSN/MSN) or Social Work (BSW/MSW) from an accredited institution.
  • An active, unrestricted RN or Social Work license (e.g., LCSW, LMSW) in the state of practice or compact license, and BLS certification.
  • Minimum 1-2 years of recent, acute care case management experience in a hospital or clinical setting.
  • Case Management Certification (CCM, ACM-RN, CMGT-BC) highly preferred; prior travel experience is a plus.

Key Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Facilitate appropriate care delivery and execute comprehensive discharge planning.
  • Coordinate timely discharge processes, securing post-acute services and community resources.
  • Apply utilization review criteria (e.g., InterQual, MCG) to ensure appropriate level of care.
  • Educate patients and families on health conditions, treatment plans, and self-care strategies.
